What happened

The Justice M.B. Shah Commission report, tabled in Parliament in September 2012, found industrial-scale illegal iron ore mining in Goa and held that the Chief Minister, who also held the mines portfolio, was aware of the non-compliance and illegalities and did not act on complaints.

Mining was suspended, then resumed under new leases. Two former chief ministers were eventually booked. Prosecutions have moved slowly and the sums the Commission identified have not been recovered. The state's mining regulation was rebuilt around the same industry it had failed to police.

The Commission's conclusions are findings of an inquiry, not convictions, and remain contested in court.

What accountability should have looked like

Ore export volumes reconciled against lease permissions in public, monthly, which is all it would have taken to see this as it happened.

Ministers and office-holders responsible

Naming who held the portfolio is a matter of public record. It is not by itself a finding of personal responsibility for any death or any rupee.
